Policies

1. New Patient Examination – a one hour appointment consisting of a complete gum examination, cancer screening, necessary x-rays, diagnosis and treatment planning.  Please arrive ten minutes early to complete a new patient medical history form.
2. Our accepted methods of payment are American Express, Visa, MasterCard, Debit Card and Cash.
3. Dental Insurance is between you and your employer.  You are responsible for paying the Dentist and your insurance company will reimburse you.  We ask that you provide the Front Desk Team with your policy and certification numbers.  Please inform the Front Desk of any changes to your policy and personal information.
4. We require 48 hours notice for cancellation of an appointment.
